Liability Waiver

This wavier must be signed by anyone riding with our club.  We need an original signature on each wavier.  It must be signed and mailed in or brought to a meeting.:

Lake Havasu Side X Side Trail Association Inc Waiver of Liability

 I recognize that riding a UTV Side x Side is an inherently dangerous activity that can result in serious personal injury or death. I/we accept the inherent risk of riding with the Lake Havasu Side x Side Trail Association Inc. including but not limited to obstacles, on and off the roads and trails, rapidly changing weather conditions, limited visibility, variation of slope and steepness on and off the trails, surfaces and sub-surfaces, conditions on and off the trails and roads, collisions with other UTV’s, including other off and on road vehicles, collisions with fixed objects including markers and signs used to mark trail boundaries or roads. Additionally, parents understand and agree that minors under the age of 18 years of age shall wear an approved DOT helmet when riding.

In consideration of my/our participation in the events and rides of the Havasu Side x Side Trail Association Inc, I/ We hereby release and agree to hold harmless and indemnify, the Lake Havasu Side x Side Trail Association Inc. their officers, directors, employees, agents, ride leaders and ride teams for all claims, injuries or liabilities caused by or created  by my/our participation. Riders must carry their own medical and accident insurance.

I/We have carefully read this agreement and release of liability and fully understand its contents. I/we are aware this release is a contract between the Lake Havasu Side x Side Trail Association Inc. and myself/us and I/We sign it of my/our own free will. My/Our signatures signify that I/We have read and agree with this release.
